Birth certificate
1.TRANSLATION OF CERTIFICATE
Translation of a document may be made either on a foreign country territory or in Poland:
•Foreign country – translations made in one’s own capacity have to be officially certified by the Consul. We require an original certificate with its translation.
•RP – translations have to be made by a sworn interpreter only.

2.REGISTRATION OF A BIRTH CERTIFICATE IN POLAND

Birth certificates of minors born abroad are registered on the basis of a written statement by one of the parents to the Vital Statistic Office, closest to his/her legal permanent residence in Poland.

If a major (over 18 years old) has never had legal permanent residence, he/she has to apply to the Vital Statistic Office in Downtown Warsaw (Warszawa-Śródmieście).

A. Personally
The following documents should be submitted in a Vital Statistic Office:
•an original birth certificate
•a translation of a birth certificate (as in point 1)

B. On behalf of persons living permanently in Poland
The following documents should be submitted in a Vital Statistic Office:
•an original birth certificate
•a translation of a birth certificate (as in point 1)
•power of attorney to register a birth certificate on behalf of an applicant

C. Sending documents directly to a Vital Statistic Office
The following documents should be sent:
•an original birth certificate
•a translation of a birth certificate
•a filed-in application to a chief of a Vital Statistic Office

D. Through the Consulate
The following documents should be submitted:
•an original birth certificate
•a translation of a birth certificate (as in point 1)
•a filled-in application to a chief of a Vital Statistic Office

Remarks
A Vital Statistic Office may require additional documents, e.g. a copy of parents’ marriage certificate.

It is suggested to use variants A, B, C rather than D because of prolonged administrative proceedings.
